For Supports Coordinators
Send referrals using the form (choose "A Supports Coordinator") or by email. Include the services, units and schedule you're looking for. We aim to reply within 24 hours with a yes and a proposed start date, or an honest no.
After a start, you'll hear from us after week one and month one, and we come prepared to every ISP meeting.

Register with IDS
In Philadelphia, people with an intellectual disability or autism register with Intellectual disAbility Services (IDS), part of DBHIDS, the city's Administrative Entity for ODP.
Choose a Supports Coordinator
You'll pick a Supports Coordination Organization. Your SC helps you find, coordinate and monitor services, and builds your ISP with you.
Ask for Jubile Care
When services are identified in your ISP, tell your SC you'd like Jubile Care as your provider. It's your choice to make.
