Services that open doors to everyday life.
Four ODP waiver services for people with intellectual disabilities and autism, plus private pay home care for anyone who needs support at home. One team, one standard.
Funded through your waiver. Shaped by your ISP.
Jubile Care provides non-residential home and community-based services under the Consolidated, Community Living and Person/Family Directed Support (P/FDS) waivers. Every service follows the waiver definitions and the ODP ISP Manual.

What a week can include
- Volunteering at a food bank, shelter or animal rescue
- Classes at the library, rec center or community college
- Gym, sports, walking clubs and art

- Exploring job interests and work experiences
Who it's for
- Young adults leaving school who need a weekday plan
- Adults who want more friends and things to do
- Anyone working toward competitive, integrated employment
How we deliver it
- A schedule built around your interests, not a building
- Activities planned and coordinated around your week
- Real relationships with people who aren't paid to be there
Good to know
- Jubile Care provides CPS in community locations; we don't operate a licensed facility at this time
- When time counts as community time, no more than 4 people are supported by us at one location
- Authorized through your ISP
Private Pay Home Care
Not on a waiver, or need more hours than your plan covers? Our private pay home care brings the same trained, background-checked team to older adults, people recovering at home, and families who need an extra set of hands.

Personal care
Bathing, dressing, grooming, toileting and mobility help, always with dignity and privacy.
Companionship
Conversation, games, walks, outings and a friendly face who shows up when they say they will.
Family respite
A few hours or a full weekend off for family caregivers, scheduled around your life.
Meals & home help
Meal planning and cooking, light housekeeping, laundry and keeping the home safe and tidy.
Medication reminders
Reminders and routines that keep medications on schedule, with changes flagged to family right away.
Errands & appointments
Groceries, pharmacy runs and company at doctor's visits, so nothing slips through the cracks.
Nurse-founded oversight
Jubile Care was founded by a Licensed Practical Nurse. Care plans are built with health in mind, and changes in condition are taken seriously.

- You choose us. Your Supports Coordinator sends a referral (or, for private pay, you call us directly).
- It's written down. The service, units and outcomes go into your ISP and are authorized before the first visit.
- A program lead is assigned. Staff are trained on your ISP, health needs, communication and backup plan first.
- Every visit counts. Staff deliver the service as written, record each visit through EVV and write a service note.
- We keep checking. Supervisors observe services at least every 3 months and review progress with you and your SC.
